This web site (“Site”) and related services are provided by Austin Regional Clinic and Covenant Management Systems, (“Austin Regional Clinic”) to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this Site.
Your privacy is of the utmost importance to us. Austin Regional Clinic will use your confidential medical information in order to provide you health care services. Austin Regional Clinic will, at all times, maintain your confidential medical information in strict confidence and will not disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law. Please review Austin Regional Clinic Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyChart that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.
All communications between you and Austin Regional Clinic using MyChart occur over a secure connection. You will receive Internet e-mail messages notifying you that new information is available in your MyChart account. These e-mail messages, however, will not contain any confidential medical information. All of the confidential medical information available to you in MyChart is protected and securely maintained by Austin Regional Clinic.
Use and Ownership
All pages within this Site and any material made available for download are the property of Austin Regional Clinic and/or its subsidiaries and affiliates. The content on the Site is provided for Austin Regional Clinic and its patients only. The words “Austin Regional Clinic” and its associated corporate logo are the trademarks and service marks of Covenant Management Systems and all other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Austin Regional Clinic or the third party that may own the trademark, service mark, or logo.
Termination of Access
Austin Regional Clinic may restrict or terminate access to MyChart at any time. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyChart without advance notice. If your use of MyChart violates these Terms and Conditions, the Austin Regional Clinic reserves the right to permanently terminate access to MyChart.
Access Code and Medical Information
MyChart Communication and Information is not for Urgent Matters and does not Constitute Medical Advice
I understand that MyChart is intended as a secure online source of confidential medical information.
The access code I receive will allow me to create a personal login name (called “MyChart ID”) and a password. By signing this, I assure ARC I will keep my login name and password completely confidential. Anyone with access to my MyChart ID and password will be able to access my or my child or proxy’s confidential medical information, read my messages, and send new messages, so I will make efforts to prevent disclosure of my login information.
I agree that it is my responsibility to select a confidential password, to maintain my password in a secure manner, and to change my password if I believe it may have been compromised in any way.
I understand that MyChart contains selected, limited medical information from a patient’s medical record and that MyChart may not reflect the complete contents of the medical record. I also understand that a paper copy of a patient’s medical record may be requested from the patient’s clinic.
I understand that my providers will share health information about me, my child or dependent, or adult for whom I am managing their health, through MyChart, including test results, and it is my responsibility to view any information released into MyChart. I also understand that with regard to sensitive test results and some abnormal test results, such results will be communicated with me or directly with the ARC patient by phone or in person and will not be released in MyChart.
Accuracy and Integrity of Information
I understand that messages sent via MyChart might not be addressed in a timely manner or might be routed to another authorized caregiver within ARC in order to facilitate a timely response.
I understand that my activities within MyChart may be tracked by computer audit and that entries I make may become part of the medical record.
I acknowledge MyChart is not a substitute for consultation with my provider and I will consult with my provider regarding my health conditions should the need arise. Any communication used through MyChart is NOT for urgent matters. For urgent issues, I will contact my physician, go to an ER, or call 911. I agree to not change or stop any course of treatment prescribed by my physician based on information found on MyChart without first consulting my provider first.
I understand that access to MyChart is provided by ARC as a convenience to its patients and that ARC has the right to deactivate access to MyChart at any time for any reason. I understand that use of MyChart is voluntary and I am not required to use MyChart or to authorize a MyChart proxy.
Although ARC attempts to ensure the integrity of the Site, it makes no guarantees as to the correctness or accuracy of the information. It is possible that MyChart could include typographical errors or other inaccuracies. Please consult your physician’s office if an inaccuracy arises.
ARC makes no representations whatsoever about any other web site that you may access through MyChart. When you access a non-ARC site, please be advised that it is independent from ARC, and that ARC has no control over the content on that web site, and ARC does not endorse or accept any responsibility for the content or use of any other linked site.
